Video about GreenLEPGreenLEP - Enucleation of the Prostate with GreenLight LaserBPH surgery has been recently revolutionized by the use of lasers. Patients can now be treated successfully with minimal risk of complications and hospital stay. Holmium laser (2140 nm) enucleation of the prostate has gained its reputation as the gold standard treatment due to its ability to remove the entire adenoma endoscopically with minimal risk of complications. Unfortunately it has not been adopted by many centers worldwide due to its technical difficulty and long learning curve. GreenLightTM PV and HPSThe advent of the GreenLightTM PV (80 watt) and HPS (120 watt) systems, with a 532 nm wavelength providing excellent ablative and hemostatic properties has made the laser treatment of BPH much more acceptable and has been widely adopted worldwide. The greenlight laser vaporization of the prostate has become a standard treatment in many hospitals and allows for the treatment of high risk patients including those on anticoagulation therapy with minimal risk of extended hospitalization, providing the possibility to treat these patients in day care or ambulatory units. Despite the safety and fantastic initial clinical results reported with these lasers, there has also been some criticism, e.g. the difficulty of removing the entire adenoma with vaporization, the consequently lower postoperative volume reduction and lower PSA reduction obtained and also the possibility of a higher reoperation rate with time. Also, some surgeons feel that the lack of a histological specimen could avoid the diagnosis of occult cancer, in comparison to Holmium enucleation or the classical TURP or open prostatectomy. Enucleative technique with the GreenLight-LaserHaving treated more than 1500 patients per sonally with the GreenLightTM laser, I decided to try to develop an enucleative technique with this laser. I felt that although treating large glands with vaporization was possible, it required the use of several laser fibers, with the consequently longer surgical times and higher procedural cost, and it still was very difficult to achieve volume and PSA reductions similar to those obtained with HoLEP. GreenLEP" or GreenLightTM-Enucleation of the prostateThe GreenLEP, or GreenLightTM enucleation of the prostate, allows for a complete enucleation of the adenomatous tissue using 80 watts, benefiting from the excellent hemostasis provided by the GreenLightTM Laser, which makes the subsequent morcellation an extremely safe procedure that can be performed under perfect visibility. The side firing fiber, far from being a drawback, can be very useful as it allows doing some initial vaporization that will set the surgical landmarks of the operation and protect the external urinary sphincter, facilitating as well the enucleation phase of the operation. Instruments for prostate enucleationOf course, a successful GreenLightTM enucleation requires the use of the best endoscopic instruments. I have been able to develop this technique thanks to these wonderful WOLF instruments that allow choosing between a 24.5 Fr. laser instrument for the smaller glands, also ideal for prostate vaporization, and a 26 Fr. instrument for the larger ones. Piranha MorcellatorI am always impressed by the speed and safety of the new Piranha morcellation system. Morcellation has been clearly improved and it is no longer a scary procedure. As a surgeon these instruments allow me to provide my patients with the best treatment for their BPH problem, irrespective of their prostatic size. We remove our patient's catheters the morning after the procedure, and they go home happy and cured.
