Arthroscopy has been a tool in standard orthopedic operations on many joints, for example the knee and shoulder joint, for many years. However, the surrounding musculature makes access to the hip joint much more difficult by comparison with the knee and shoulder joints. In recent years, a number of surgeons have refined hip-joint surgery and developed techniques which permit arthroscopic treatment of an increasing number of conditions associated with the hip joint. Hip joint arthroscopy is a procedure which allows the joint cavity between the head of the femur and the socket of the hip bone to be inspected, as well as the rest of the joint cavity. The main reasons for hip joint arthroscopy are pain in the hip joint. This pain can be due to a number of different causes. Frequent indications for hip joint arthroscopy are free arthroliths in the hip joint, impingement of the hip joint, cartilage damage in the hip joint, disease of the joint mucus membrane in the hip joint, and damage to the acetabular labrum.

In cooperation with Dr. Miehlke (ARCUS Sports Hospital, Pforzheim), we have
developed a special Instrument Set in order to make this kind of operation as
easy as possible and ensure that it can be carried out with maximum safety.

In recent years there is an increasing trend for full endoscopic hip procedures. As well as for diagnostic use new arthroscopic techniques to treat a huge range of hip diseases like lesions of the acetabular labrum, the impingement syndrome or extraction of foreign bodies have been developed and set as a standard.

In cooperation with Dr. Wolfang Miehlke from the ARCUS Sports Hospital Pforzheim / Germany the "Coxartis" instrument set  has been created that, due to its easy
handling and ergonomic design, can be applied by experienced surgeons as well as by novices in hip arthroscopy.

Having been developed in strict accordance with our customers demands, this instrument set is perfectly designed for the special anatomic needs of the hip joint.

Stable puncture cannulas allow direct passage through all soft tissue structures for the primary access to the joint under X-ray control and the creation of other access portals under endoscopic control. The semi flexible guide wires made of Nitinol (Ø 2 mm) are introduced through the inner lumen of the puncture cannulas into the joint and serve as a blunt guide rail for the introduction of all cannulated instruments.
As the first dilator, the cannulated rod expands the access portal to the joint over the guide wire. The trocar sleeves of the arthroscope can be introduced directly over this rod. The semi dish shape of the guide tube is used as a guide channel for introducing instruments. This can be introduced into the joint either via its cannulated obturator or via a dilator (blue).

Dilation system

The user-friendly dilation system consists of three colour-coded "one-step" dilators, each with a different diameter, that are introduced into the hip joint one after the other starting with the smallest diameter using the ergonomic handpiece attachment.

Arthrolution-Arthroscopes

The Arthrolution trocar sleeves were shaped specially for atraumatic use in the joint in conjunction with a PANOVIEW telescope with a 70° angle of view. Telescopes with a 30° or a 45° angle of view can also be used. A maintenance-free click locking mechanism guarantees simple, safe and fast connection of the telescopes in the trocar sleeves.

Cannulated trocars allow direct introduction of the arthroscopes into the joint via the guide wire. A new continuous-irrigation arthroscope was developed to optimise the view in the joint. Balanced continuous irrigation and suction allow the joint to be irrigated quickly and guarantee an excellent view.

Longer arthroscopes are also available for obese patients.

Manual instruments

  • Specially shaped curettes with ergonomic handpieces for effective removal and smoothing of bone, remnants of cartilage and calcium deposits.
  • Extended hooked probe, graduated for palpation and measuring.
  • Rasp for debriding and removing bone.
  • Banana knife for incision of the joint capsule
  • Punches: Sharp punches with adapted length and various degrees of bend and angle used to resect dense tendinous and ligamentous structures.

A hip arthroscope was developed in close cooperation with Prof. Chiron, Toulouse. The special construction of the endoscope with an integrated working channel enables the intervention with only a single access.

Mechanical and motor driven instruments, as well as the use of laser probes, allow a very broad application range.
