ACL Reconstruction - Arthroscopic reconstruction of the anterior cruciate ligament

Reconstruction of the ACL using the hamstring technique has recently become of increasing interest. Beside this allows the product range of Richard Wolf the ACL reconstruction with the following tendons and fixation techniques:

  • ACL reconstruction with Patella Tendon. Fixation in Pressfit-technique or Bioactive screw
  • ACL reconstruction with Quadrizepssehne. Fixation in Pressfit-technique or Bioactive screw
  • ACL reconstruction with Hamstring- Semitendinosus- Gracilissehne. Fixation in Pressfit-technique or Bioactive screw
ACL reconstruction with Bone-Tendon-Bone technique using patellar tendon

Fixation close to the insertion

Fixation close to the insertion has been suggested and methods have already been used successfully:

  • Fixation using an interference screw
  • Suspension of loops using transverse rods

Disadvantages of these techniques are:

  • High hardware costs
  • Problems with revision surgery
  • Awkward and time-consuming

Pressfit technique

ACL reconstruction- with Bone-Tendon technique using quadriceps tendon
ACL reconstruction- with Bone-Tendon technique using quadriceps tendon

Alternative solution the Pressfit technique has been developed with the following major features:

• Fixation close to the point of insertion
• Avoidance of hardware
• Simple graft preparation

ACL reconstruction with Endobutton

Fixation using endo buttons the most common method. The disadvantages of this away from the point of insertion are:

  • enlargement of the tunnel
  • creep at the tendon-tape transition along with giving tendon construct and permanent elongation. This is also known as the Bungé effect.
